Healthcare Document Uploads: Patient Matching, Provenance and Review States
Design healthcare uploads with patient association, source provenance, technical checks, review states and controlled correction history.

Healthcare document uploads should identify the intended patient and encounter, preserve source information and show whether a document is unreviewed, accepted or requires correction. Receiving a file does not verify its contents or establish a clinical interpretation. Define those boundaries with the institution's authorised teams.

Let’s plan your healthcare document flow.
Bring your workflow and the decisions you need to make. We’ll help you clarify requirements, review the trade-offs and define a practical next step.
Capture the minimum useful context
Agree document type, source, relevant date and intended record association. Distinguish the date on a document from the time it was uploaded. Preserve the original file where required by the approved records process, and identify any transformed preview separately.
Avoid matching solely on a filename or typed name. Give authorised staff a controlled way to investigate ambiguous associations without exposing other patients' records to the uploader.
Specify document handling states
| Stage | Requirement | Acceptance scenario |
|---|---|---|
| Selected | Explain supported files and limits | Incorrect format rejected clearly |
| Received | Track upload reference | Retry does not create misleading duplicates |
| Checked | Apply agreed technical checks | Rejected file stays unavailable for normal use |
| Unreviewed | State that professional review has not occurred | User does not see a false reviewed badge |
| Associated | Confirm patient and record relationship | Authorised correction preserves history |
| Superseded | Identify replacement or newer version | Current document is distinguishable |
Technical scanning, record association and clinical review are different controls. Do not combine them into one Approved status without defining exactly what that status means.
Preserve provenance through transformations
A hypothetical photographed report may be rotated or processed for text extraction. Retain the relationship between the original and any derived content. If extraction is uncertain, route it for the agreed review instead of treating inferred text as a verified source record.
For broader governance, ONC's SAFER guides include patient-identification and communication topics. They are useful review material, not evidence that this checklist establishes compliance with Indian or other local requirements.
Test access and correction
Try an incorrect patient association, an expired session, a repeated upload and a document replaced after review. Verify download permissions and what appears in notifications and logs.
Agree the authorised deletion, retention and correction processes with records and policy owners. The upload screen should explain its actual behaviour rather than promise that every downstream copy can be removed instantly.
Document workflow questions
Can OCR make an uploaded report immediately authoritative?
No. Extracted text needs a defined verification and provenance model appropriate to its use.
Should patients see every internal review note?
Define visibility through the institution's approved policy and role model rather than exposing all stored metadata by default.
Use the mobile upload checklist for transfer behaviour, and review the records workflow with Drema's healthcare team.
