Patient intake software should distinguish information entered by a patient from information reviewed or accepted by authorised staff. Preserve the source, relevant version and correction history. Submitting a form does not establish that its clinical contents are verified or that a professional has acted on them.

Let’s plan your patient intake flow.
Bring your workflow, constraints and open questions. We’ll help you define the requirements and work through a practical next step.
Define the form's purpose
Have the institution's responsible teams decide which administrative and clinical information is needed, who may provide it and how it enters the official record. Keep clinical content and decision rules under qualified governance.
For a hypothetical clinic, contact details may be handled differently from a patient-reported history. The interface should not apply one undifferentiated Verified badge to both.
Use an intake lifecycle worksheet
| Stage | Meaning | Requirement to test |
|---|---|---|
| Draft | Information is incomplete | Clear save and resume limits |
| Submitted | User has sent a version | Stable reference and source identity |
| Needs clarification | Specific information requires attention | Assigned question and response route |
| Reviewed | Defined staff review occurred | Actor, time and scope |
| Corrected | Earlier information changed | Original and revised meaning traceable |
| Incorporated | Approved data enters the owning record | No unexplained duplicate entry |
Define which changes require renewed review. A later answer should not silently replace information staff relied on without the approved notification or reconciliation process.
Make uncertainty understandable
Provide an appropriate way to indicate unknown or not applicable where the clinical and administrative design permits it. Forcing an arbitrary answer to proceed can make the dataset look complete while reducing reliability.
Explain what the form is for and how it will be reviewed. Do not imply that an automated acknowledgement means a clinician has assessed the submission.
Test identity and interrupted entry
Use synthetic accounts to test proxy entry, changed appointments, expired sessions and staff corrections. Verify the intended patient and encounter context before accepting the form.
Restrict draft access on shared devices and account changes. Display useful validation without exposing previously entered sensitive information to another user.
Intake questions
Should patient-entered data immediately overwrite the main record?
Use the institution's approved review and reconciliation process for each information type.
Can a completed form be treated as clinical clearance?
Not from form completion alone. Clinical authority and interpretation require a separately governed process.
Connect intake to proxy-access design and form recovery. Discuss the workflow with Drema's healthcare team.
